Our man deals here with a local concern with his local community. I do not see "heresy" in this letter; by extension, the target is not the Had-Qnoma. (So I agree with Bcheiry that this menace is yet to arrive.) The "worm" has come from within the community itself.
One more vital point: it's addressed straight to the top, to Isho'yahb II (wherever he is); and not to Arbela - nor to Mar Gabriel in Kirkuk. There was peste en Bet-Garmay.
